Mainstream Physical Therapy is Medicare certified and participates with most major local insurance and
managed care companies. See the below list of frequently used providers, to check for your insurance carrier.
If you do not see your provider on the list, please contact our office at (239) 415-2595 to see if they have
been added recently, as contracts change frequently.
Staff members file primary insurance for patient convenience and will assist patients with their questions
and other third party payer needs.
